      <description><![CDATA[<p>This position may qualify for up to a $2,000 retention bonus! The Wachusett Respiratory Care Unit at Holden Rehabilitation and Skilled Nursing Center has openings for a full time<span>&nbsp;Certified Nursing&nbsp;</span><span>Assistant.&nbsp;&nbsp;</span>The Respiratory Unit is a specialty unit providing care to patients with ALS, Traumatic Brain Injury and Ventilator Dependency.&nbsp;&nbsp;<span>The hours are 7A to 7P, 37.5&nbsp; hours per week and includes every other weekend.</span>&nbsp; The desired candidate will have a passion for caring for residents needing 24 hour critical care in a long term care environment. &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</span>Educational: State certification required within 120 days of hiring.&nbsp; CEUs (Continuing Education Units) as required to maintain certification.</p><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</p><p><b>Skills:</b><br></p><p><span>&nbsp;</span>&nbsp; &nbsp; &nbsp; Ability to follow assignments.&nbsp; Ability to speak, read, and write English.</p><p><span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Ability to maintain assigned work hours.&nbsp; Has sufficient endurance to perform&nbsp;</span>tasks over a long period of time.</p><p><span>&nbsp; &nbsp; &nbsp;&nbsp;</span>Ability to perform the essential functions:</p><p><span>&nbsp;</span></p><p><span>In this position, the employee is required to be on his or her feet continually&nbsp;</span>throughout the day.&nbsp; On a full time, nonrestrictive basis, the employee must be able</p><p><span>to lift frequently over 50 lbs. of weight, which is required when transferring a&nbsp;</span>resident from a bed to a wheelchair.&nbsp; The use of a transfer belt is necessary and</p><p><span>required when transferring any resident.&nbsp; This is for the safety of not only the&nbsp;</span>resident, but also the Nursing Assistant.&nbsp; Other Nursing Assistants or devices are at</p><p><span>times available to assist with heavier residents if needed.&nbsp; Must have the ability to&nbsp;</span>perform essential functions without posing a â€œdirect threatâ€ to patients or the staff.</p><p><span>&nbsp;</span><b>Essential Functions:</b></p><p>1.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Assists residents routinely and as needed with any and all activities of daily living, including oral hygiene, bathing, grooming, dressing, toileting, nutrition and feeding,</p><p><span>&nbsp; &nbsp; &nbsp; &nbsp;exercise and mobility, transfer and ambulation, transportation and activity.</span></p><p><span>&nbsp;</span>2.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Receives instructions and guidance from the licensed nurse(s) on the unit regarding individual resident needs.</p><p><span>&nbsp;</span>3.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Consults residentâ€™s written plan of care to obtain information and instructions regarding individual residentâ€™s needs.</p><p><span>&nbsp;</span>4.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Performs assigned tasks according to established procedures and practices including:</p><p><span>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</span>a.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Gathering and recording data, e.g. â€“ vital signs, weight, intake and output, specimen collection, etc.</p><p><span>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</span>b.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Carrying out nursing routines. e.g., to prevent decubiti, contractures and&nbsp;&nbsp; immobility and to keep patients clean, comfortable, safe and secure, etc.&nbsp;</p><p><span>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; c.<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; </span></span><span>Providing restorative nursing care, e.g., range of motion, positioning,&nbsp;</span>preventative skincare, reinforcing teaching and providing assistance with activities of daily living, exercise and activity, etc.</p><p><span>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</span>d.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Assisting in maintaining the safety and cleanliness of the residentâ€™s total environment.</p><p><span>&nbsp;</span>5.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Makes observations of residentâ€™s physical and mental state and reports observations and changes to nurse in charge including mental status, behavior/mood, vital signs, weight, skin condition,&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;mobility status, appetite/nutrition, activity level, level of independence in activities of daily living, and progress in restorative nursing programs.</p><p>6.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Reports and records accurately all current resident observations and information in residentâ€™s records and on other forms as provided.</p><p><span>&nbsp;</span>7.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Communicates with appropriate staff regarding all aspects of resident care and seeks assistance as necessary.</p><p><span>&nbsp;</span>8.&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
